Who on Your Team Can Respond — and to What
The most common gap we see is not a missing protocol. It is a protocol that never says who is authorised to act. A written procedure that does not name the responder is not an operational document — it is a description of what someone, somewhere, ought to do.
Emergency response splits into three tiers, and your protocols should assign every action to one of them:
- Anyone on site, including front-of-house. Recognising distress, calling for help, activating 911, retrieving the emergency kit and the AED, clearing the corridor, and meeting paramedics at the door. None of this requires a clinical licence, and all of it is time-critical. Reception staff are frequently the first to notice a patient in trouble in the waiting area.
- Trained clinical staff. Taking vital signs, positioning the patient, administering oxygen where available, and initiating the specific response your medical director has authorised in a standing order — for example epinephrine for anaphylaxis. The scope here is set by licence type and by what the standing order actually says.
- The prescriber or medical director. Decisions that constitute practising medicine: hyaluronidase dosing for suspected vascular occlusion, prescribing, and the call on whether to transfer. Your protocol must state how this person is reached within minutes, and what happens when they are not immediately reachable.
That last point is where practices get cited. "Immediately reachable" is a real standard in several states, and an unanswered phone during an adverse event becomes part of the record. What Your Emergency Kit Must Contain
A protocol that calls for a medication you do not stock is worse than no protocol, because it documents that you knew what was needed and did not have it. Core items for an aesthetic practice are:

What to Document After an Event
Post-incident documentation is written for two audiences you hope never to meet: a regulator and a malpractice defence attorney. Both will ask the same question — what happened, and when. Record:
- The exact timeline of events, with clock times rather than durations
- Presenting symptoms and vital signs
- Every treatment administered, and by whom
- The 911 activation time, if applicable
- Hospital or specialist follow-up and the patient outcome
- A root cause analysis and the corrective action taken
Write it the same day. Reconstructed timelines are visibly reconstructed, and a gap between the event and the note is the first thing an opposing expert will point at.
What an Inspector Looks For
Emergency preparedness is one of the first things examined when a state board arrives, and it is examined through documents rather than conversation. Expect the inspector to ask for your written protocols, confirm they carry your medical director's signature and a current review date, check that standing orders authorise what your staff actually did, and pull training records to see whether drills happened.
